1. All patients shall receive
a copy of the HOPEWELL CENTER PATIENT'S RIGHTS AND RESPONSIBILITIES upon
admission. The case manager shall review the document with the patient
or guardians and answer any questions they may have.
2. The PATIENT
RIGHTS AND RESPONSIBILITIES will be clearly posted on every floor in all
areas accessible to patients.
Some Client Rights
are listed under separate heading because they are subject to
limitations due to safety considerations, treatment requirements, or to
meet requirements of State and Federal laws (e.g. Federal HIPPA privacy
regulations). The conditions under which these rights can be limited are
discussed below:
1.
TO BE PAID FOR WORK UNRELATED TO TREATMENT.
An individual may be expected to
perform limited tasks and chores within the program that are designed to
promote personal involvement and responsibility, skill building, or peer
support. Any tasks beyond routine care and cleaning of activity or
bedroom areas within the program must be directly related to recovery
and treatment plan goals developed with the individual.
2.
THE RIGHT TO SEE AND REVIEW YOUR OWN TREATMENT RECORD, SUBJECT TO
FEDERAL AND STATE GUIDELINES AND REGULATIONS.
A request from a client to review
records must follow the Hopewell Patient Information Privacy Policy and
the Procedure for Client Access to Records.
3. The following
rights are subject to limitation only to ensure the personal safety of
clients or others. Any limitation due to safety considerations shall
occur only if it is:

Applied on an individual
basis
·
Authorized by the
President / CEO or designee
·
Documented in the
individual's record
·
Justified by sufficient
documentation
·
Reviewed on a regular
basis at the time of each individual treatment plan review
·
Rescinded at the
earliest clinically appropriate moment
