Responsible for providing activities and interactions that enable an individual at clear risk of hospitalization or who has discharged from inpatient treatment to maintain an adjustment to living in the community. Responsible for assessing the client's status including personal strengths, progress, problems and needs. Maintains client charts and assures the standards and guidelines are followed. Education: Bachelor's degree in Social Work, Psychology, Nursing or related field. Must have valid driver's license, dependable automobile and personal vehicle insurance. |

Responsible for the overall planning, organization and supervision of the Health Information Services Department, including Appointment Processing, Patient Registration and Reception area for the organization. Tracks supplies to ensure that supplies and equipment are available as needed. Ensure that policies and procedures are developed and followed as they pertain to medical records. Education: 5 plus years experience medical records, registration or call center operations. Excellent customer service skills required. |
| Outpatient Case Manager |
The primary role is to serve as a liaison outreach worker for patients in the outpatient program who are not complying with treatment, i.e. failing to keep appointments. The case manager will also assist patients in transitioning from inpatient to outpatient service or a lesser level of care. Will provide therapy, screening and crisis intervention. Education: Education: Must have a Master's Degree in Social Services (Counseling, Social Work, Psychology, or Sociology). Two (2) years experience working with the mentally ill population. Knowledge of treating patients with co-occurring disorders. Prefer certification in substance abuse and/or licensure as a Professional Counselor or Clinical Social Worker. |
| Supervisor, Community Support Specialist |
Will provide detailed reviews of client situations and necessary interviews, identification of referral needs, review and adequacy of service delivery, and time management. Supervisors will maintain a log of these sessions for each of their assigned workers with observations that will be used in the worker's annual performance evaluations. Perform clinical interviews for intake and periodic evaluations of CPRC clients as prescribed by CPRC policies and procedures. Conduct record audit within the quality assurance process Education: Master's degree form an accredited university in Psychology, Counseling, Social Work or Psychiatric Nursing and two (2) years of relevant experience with the treatment and assessment of the mentally ill, Experience in supervision preferred. |

Responsible for care management involving medication treatment plans, patient teaching, counseling and assistance as needed. Confers with pharmacy, sales representatives and other health care providers to coordinate various activities. Administers all psychotropic (intramuscular, oral and subcutaneous) per psychiatrists' orders. Coordinates activities for patients referred to other community services. Coordinates scheduling for laboratory tests, physical exams, and diagnostic services within and outside the clinical site. Completes AIMS testing, Med Profiles, Lab Reviews, and metabolic screenings as needed. Follow-up prescription refills and prior authorizations. Education: Licensed Practical Nurse. Prefer experience working with mental health and strong computer skills. Licensed with the State of Missouri. |

Plans and coordinates all psychiatric aspects of patient care in Primary Care and Behavioral Health environments. Completes and documents thorough psychiatric evaluations. Work towards developing integrative care as it pertains to medical/psychiatric services. Acts as liaison person with local medical agencies to ensure continuity of care. Licensed by the State of Missouri to practice medicine. Medical degree, residency in psychiatry. |
