The Following is a Listing of the Services We Provide Our Adults:

Residential Care Facility

Goal: The ALM Hopewell Center provides housing for adults with a Serious and Persistent Mental Illness.

Eligibility:

  • Individuals who have been diagnosed with a Serious and Persistent Mental Illness
  • Must be at least 18 years old

Services Provided:

  • Addresses the issues of consumers by providing skill-based programs that will aid consumers when they are reintegrated into the community
  • Programming includes health and medication education as well as daily living skills.


Outpatient Services

Outpatient psychotherapy is the least intensive and least restrictive treatment modality contained within the clinical repertoire of mental health treatment interventions. Outpatient psychotherapy recipients continue to participate in both family and community activities and receive encouragement to participate in many activities that emphasize strengths and increase in level of functioning -- and discourage isolation. Individual sessions may cover the follow:
  • Anger & Assertiveness
  • Depression & Empowerment
  • Beyond Recovery
  • Grief & Loss
  • Emotional Well-Being
  • Anger Management
  • Anxiety
Goals for consumers
  • Stabilization of psychiatric symptoms or acute crisis
  • Return to premorbid level of functioning
Components
  • Initial intake assessment -- Must include diagnosis, previous treatment, relevant history, current medical conditions, including medications, substance abuse history, risk of harm to self or others, and a complete mental status exam
  • Individualized treatment plan
  • Planned brief psychotherapy
  • Quality Assurance (QA) and Utilization Review (UR)
  • Appropriate clinical documentation in an individual's care record

Psychiatric Services

A trained staff of professionals, which include Psychiatrist, Mental Health Nurse Practitioners, Registered Nurses and Licensed Practical Nurses are available to provide or assist with
  • Psychiatric Evaluations
  • Medication Management
  • Medication Administration
  • AIMS (Abnormal Involuntary Movement Scale) Testing
  • Metabolic Screenings

Psychosocial Rehabilitation Services (PSRC )

Top
are primarily delivered in a group setting. These services develop, support and optimize the quality of life and functional abilities of people with serious and persistent mental illnesses. Such programs have a commitment to community integration.
Psychosocial rehabilitation service provides structured and purposeful activities where individuals can engage in a group/community type setting with peers and staff in a way that facilitates community integration. Click here to read more.

Services may include:
Structured purposeful activities; groups for budgeting, skill training or ADL training to support independent living
  • Socialization and use of leisure time services
  • Vocational and pre-vocational rehabilitation services
  • Crisis intervention services
Goals
  • Improve or sustain the member's level of functioning
  • Increase the member's ability to cope with his or her psychiatric disability
  • Reduce the need for hospitalization
Admission Criteria
  • Have a serious mental illness as primary diagnosis
  • Have documented functional deficits
  • Has an expressed desire to learn social skills or enhance the quality of relationships
  • Has an expressed desire to learn pre-vocational skills

Community Psychiatric Rehabilitation

Top The Amanda Luckett Murphy Hopewell Center main focus is to improve quality of life for persons with serious mental illnesses so that they can lead productive and meaningful lives in the community. Through the Community Support Program we strive to :
  • Prevent client hospitalizations
  • Promote independent living
  • Assist with community re-integration
  • Help clients develop support systems within the community
  • Teach clients ways to manage symptoms of their illnesses
  • Provide education and support to families

Community Support Services we provide to mentally ill adults within the community:


  • 24-hour crisis response
  • Intensive case management
  • Day treatment, emphasizing community living skills
  • Community re-integration including independent living, pre-vocational and leisure skills training
  • Supportive community living through our residential facilities or semi-independent apartment living
  • Psychiatric services, including medication management and administration
  • Support groups for clients and families
  • Transportation
  • Assistance with community integration, creating supportive relationships and overcoming fear of others
  • Assistance with establishing life and individual recovery goals, e.g. school, work, parenting
  • Assistance with managing Medicaid, Social Security and other public assistance
  • Social skills training
